2025 MIPS Proposed Rule is trying to make it easier on practices

CMS released its Proposed Rule for MIPS in 2025. While there were not any changes in the composite score breakdown – you will still need 75 points to avoid a penalty. Not reporting can mean a 9% penalty – which could have a significant impact on your practice.
Some of the changes in each category are highlighted:
Improvement Activities: the Improvement Activity inventory will be reduced to 100 available choices, down from 106. But CMS is proposing to also reduce the number of activities to be reported, by removing the weighting classification (medium-weighted or high-weighted) and reducing the number of activities to be reported. This should reduce the complexity with the scoring, and hopefully, help make your reporting a little easier.
For example, if you are a large practice (16+ providers), you will be required to report two activities while small practices, rural or non-patient facing practices, and those reporting using MVPs, will only need to complete one IA activity. This should make it less stressful to figure out potential measures and how their scores will be adjusted when you attest to them.
CMS is also adding language around how they will handle multiple submissions from either different organizations or multiple from within the same organization. I you submit multiple IA activities, your most recent submission will override any previous submissions.
Promoting Interoperability: there are no proposed changes to the objectives for 2025. If you have been reporting on MIPS from the initial start, your practice is aware of what the criteria is for submissions.
CMS is proposing policies which will require minimum submission criteria, and policies which address multiple data submissions – many of which your practice is performing with your current submissions. The minimum submission criteria are available to view in a recent webinar held in early August 2024, viewable below.
